Position Title: Community Nursing Care Support Worker

Position Overview
The Community Nursing Care Support Worker provides essential nursing and healthcare services to NDIS participants. This role covers a range of health-related supports, including assistance from Enrolled Nurses, Registered Nurses, Clinical Nurses, Clinical Nurse Consultants, and Nurse Practitioners, ensuring participants receive appropriate care for their medical and personal needs.
Key Responsibilities
· Provide community nursing care tailored to participant needs.
· Administer medication and treatments in accordance with healthcare plans.
· Assist with wound care, continence management, and chronic disease management.
· Work collaboratively with healthcare professionals and support teams.
· Monitor participant health status and escalate concerns when necessary.
· Deliver nursing support at home or in community settings as required.
· Educate participants and their families on health management strategies.
· Maintain accurate clinical records and adhere to NDIS compliance requirements.
Skills & Qualifications
· Qualification as an Enrolled Nurse, Registered Nurse, Clinical Nurse, Clinical Nurse Consultant, or Nurse Practitioner.
· Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
· Experience in community nursing, disability support, or aged care.
· Strong clinical assessment and critical-thinking skills.
· Effective communication and interpersonal skills.
· Current First Aid and CPR certification (if applicable).
· Working with Children Check and NDIS Worker Screening Check (if applicable).
Physical Requirements
· Ability to provide direct care, including lifting and repositioning participants as required.
· Comfortable working in various home and community environments.
